Terms and Conditions of Service
These Terms and Conditions of Service will apply to the purchase of the services by you (the Client or you). We are Summers Consulting Services Ltd whose trading name is Anni Summers RD a company registered in England and Wales under number 13673662 whose registered office is at 4a Stansted Courtyard, Parsonage Road, Takeley, Bishop’s Stortford, CM22 6PU with email address firstname.lastname@example.org; (the Supplier or us or we).
These are the terms on which we sell all Services to you. Before entering into this Contract with us, you will be asked to agree to these Terms and Conditions of Service by clicking on the checkbox marked ‘I hereby agree to the document above’, as part of your onboarding process. If you do not click on the checkbox, you will not be able to complete your Order. You, the Client, can only purchase our Services for your own use and not for any other person. You must be eligible to enter into a Contract and be at least 18 years old to purchase our Services.
Client means an individual acting for purposes which are wholly or mainly outside their trade, business, craft or profession;
Contract means the legally-binding agreement between you and us for the supply of the Services;
Order means the Client’s order for the Services from the Supplier
Services means the Services we supply to you;
The description of the Services is as set out on our website (www.annisummers.com), social media accounts, catalogues, brochures or other forms of advertisement. Any description is for illustrative purposes only and there may be small discrepancies in the content of the Service supplied. All Services are subject to availability. We can make changes to the Services which are necessary to comply with any applicable law or safety requirement. We will notify you of these changes.
We, the Supplier, connect with the Healthie Patient Management System (Healthie) platform to store personal data provided to us by you, as well as carry out our Services. You will be given access to the Healthie online portal, through which to can complete forms and diaries, view consultation summaries, book sessions and make payments.
We do not, as standard, provide you with a detailed nutritional analysis of your diet, nor do we supply you with meal plans to follow.
All Services are delivered remotely, using video conferencing software, and are referred to as telehealth services or telehealth consultations. This differs from face-to-face consultations, as we will not be in the same room as each other during the consultation. You will need a stable internet connection, microphone and webcam to use telehealth services.
Video recordings are made of telehealth consultations. The sole purpose of this is to facilitate information collection whilst ensuring the uninterrupted flow of the consultation, enhancing your telehealth experience. A transcript is generated and used for my reference to ensure that the information provided during the consultation is accurately documented in your clinical record. Recordings and transcripts will be stored in line with UK GDPR guidelines and destroyed in most instances within 48 hours, and certainly no later than one week, after the date of the recording.
Parts of the consultation involving physical examinations (e.g. body weight), will be conducted by you at your location, with you being responsible for the accurate measurement and reporting of the results of any such examination.
The main risks related to telehealth consultations include: technical difficulties, interruptions, unauthorised access and communication difficulties.
We will ensure that your privacy is protected at our location during telehealth consultations by ensuring nobody else can hear the consultation and using secure and encrypted software. You are responsible for ensuring your privacy is protected at your location.
If an issue arises with technology which we can not fix during the consultation, we can continue the consultation by telephone using the telephone number you provided us with, or rearrange the consultation if needed.
Summers Consulting Services Ltd is registered with the Information Commissioner’s Office (ICO) as a Data Controller, registration number ZB360661. Anni Summers is the Data Protection Officer.
All Client details, clinical notes and correspondence will be stored securely and treated confidentially according to General Data Protection Regulations (GDPR) and the Data Protection Act 1988. Information is stored in accordance with The Health & Care Professions Council (HCPC) standards.
The initial consultation will be agreed between Summers Consulting Services Ltd trading as Anni Summers RD and you, the Client, and you will receive an e-mail confirming this. The assessment will be undertaken and a treatment plan agreed. The need for further treatment will be communicated and agreed. A written report will be generated and shared with you, and any other parties (e.g. GP or consultant), as required, with your prior consent.
Review appointments will be agreed in advance and confirmed via e-mail. If you book a block of sessions, these can be taken over any reasonable time period as agreed between Anni Summers RD and the Client. Future sessions will be booked at the end of the session. A report or action points (as appropriate) will be generated and sent to you, and any other parties, as required, and with your prior consent.
You agree to provide accurate and complete information when you register with, and as you use Healthie, and you agree to update your account information to keep it accurate and complete. You agree that Healthie may store and use the information you provide for use in maintaining and billing fees to your account, as well as storing your personal data. You are responsible for maintaining the confidentiality of your account and password, including but not limited to the restriction of access to your computer and/or account. You agree to accept responsibility for any and all activities or actions that occur under your account and/or password, whether your password is with our Service or a third-party service. You must notify us immediately upon becoming aware of any breach of security or unauthorised use of your account.
You may not use as a username the name of another person or entity that is not lawfully available for use, or a name or trademark that is subject to any rights of another person or entity other than you, without appropriate authorisation. You may not use as a username any name that is offensive, vulgar or obscene.
Fees and Payment
Fees will be agreed upon booking appointments.
Pre-payment is required for all bookings. You will receive an invoice by email and also via the Healthie online portal.
Invoices are payable no less than 48 hours prior to your appointment.
The following process will apply in the event of non-payment: Your appointment will be cancelled. You will need to contact Anni Summers RD by telephone or email (email@example.com) to rebook.
We require 48 hours’ notice for cancellations. Cancellations within 48 hours of the appointment are chargeable at 100% of the cost of the appointment.
Failure to Attend
If you fail to attend an appointment without giving us prior notice, this will be regarded as a DNA (did not attend). DNA will be charged at 100% of the cost of the appointment.
For administrative queries such as appointment rescheduling, e-mail (firstname.lastname@example.org) is the preferred method of communication.
For clinical queries, please use the Healthie online portal, signing in using your unique username and password.
Email is never 100% secure. By emailing us, you are acknowledging this fact, and consent to us responding to this or future email communications via the e-mail address you provided us with.
